Ruptured sigmoid diverticulitis with pneumoretroperitoneum

Pneumoretroperitoneum extending from the lesser sac to the mid sigmoid colon. At this point there is some fat stranding in close relation to a sigmoid diverticulum. There is a tiny amount of fluid in the retroperitoneum inferiorly but no walled off collection or abscess.
Case Discussion
Ruptured diverticulitis causing pneumoretroperitoneum is uncommon, with the descending colon more common than the sigmoid colon.
