Diagnosis almost certain

There is a relatively well-defined lobulated and eccentric lucent lesion within the tibial epiphysis with a thin peripheral sclerotic rim.

mri

On MRI sequences the lesion appears well-circumscribed with lobulated margins, of intermediate signal on T1, high signal on STIR with heterogeneous enhancement on postcontrast sequences. No transphyseal or transcortical extension. Edematous infiltration with enhancement of the Hoffa fat pad.

Case Discussion

The patient's age and the radiographic and MRI features are suggestive of an epiphyseal lesion most likely a chondroblastoma which was confirmed after surgical curettage with histopathological exam.

For the differential diagnosis of the epiphyseal lesions (see differential for an epiphyseal lesion).
