Presentation

Known sickle cell disease, presented with left knee pain.

Patient Data

Age: 25 years
Gender: Female

There is an ill-defined serpiginous lucent lesion in the proximal tibial metaphysis with no significant peripheral sclerosis.

Extensive intramedullary lesions involving the distal femur and proximal tibia with double peripheral serpiginous lines well-visualized on T1 and PD fat saturation sequences. Hyperintense inner line of granulation tissue and a hypointense outer line of sclerosis with surrounding bone marrow edema. A cortical breach is noted at the lateral tibial plateau.

Case Discussion

The MRI features are most consistent with extensive bone infarcts in a patient with sickle cell disease.
