Presentation

Diabetic foot ulcer with extensive ulceration. To exclude osteomyelitis.

Patient Data

Age: 40 years
Gender: Female
mri

Mortise and subtalar joints effusion

Extensive bone marrow signal changes (T1 hypointense, T2, and PD hyperintense) involving the calcaneus compatible with osteomyelitis

A sinus tract starts from the calcaneus base into the skin on the plantar aspect medial side

Abnormal bone marrow signal isointense on T1 and hyperintense on T2/PDFS images involving the midfoot bones is suggestive of osteitis

Extensive edema and fluid signal intensity also are seen within the midfoot and hindfoot intrinsic muscles and subcutaneous tissues

Fluid signal around the ankle posteromedial tendons in the background of tenosynovitis

Talonavicular joint marginal osteophyte formation.
