Presentation

Painless gradually swelling on the posterior aspect of the right ankle for the last 9 months. The swelling had progressively increased in the last three months with difficulty in walking.

Patient Data

Age: 30 years
Gender: Female
mri

There is a 32 x 50 x 60 mm well-defined, lobulated heterogeneous signal (intermediate to hypointensity on T1-weighted and T2-weighted images) soft tissue enhancing mass lesion with adhesion to Achilles tendon with anterior and inferior extensions toward the ankle joint, calcaneus, and posterior ankle tendons.

Infiltration of Kager fat with underlying bony erosion of the posterosuperior calcaneus bone also is noted.

Case Discussion

Soft-tissue mass arising from the Achilles tendon inducing calcaneus bone cortical erosion. The major differential diagnosis would be between giant cell tumor of the tendon sheath and sarcoma.
