The study reveals extensive diffusion restriction involving bilateral internal capsules, bilateral periventricular, deep, and subcortical white matter with sparing of the cortex. T2/FLAIR hyperintensities are seen in basal ganglia with no diffusion restriction.
Case Discussion
The imaging pattern on diffusion-weighted images is called the 'bright tree' appearance, and the findings are pathognomonic for acute leukoencephalopathy with restricted diffusion (ALERD).
