Adenoid cystic carcinoma of lacrimal gland

Presentation

Painful right orbital swelling.

Patient Data

Age: 65 years
Gender: Male

Large lobulated right orbital soft tissue mass of extraconal location centred on the lacrimal gland. It displays an isosignal to the cortical grey matter on T1, T2, FLAIR and DWI with vivid enhancement on postcontrast sequences. No calcification or haemorrhagic component was seen on the GE sequence. It compresses and displaces the ocular globe and optic nerve with non-axial proptosis grade 2 as well as the lateral and superior rectus muscles. There is also posterior extension involving the sphenoid bone (greater wing) with extra-axial anterior temporal soft tissue component extending to the ipsilateral cavernous sinus as well as an extension to the temporal fossa invading the temporalis muscle.

MRI features of a large vividly enhancing orbital mass of extraconal location centred on the lacrimal gland with extraorbital extension invading the sphenoid bone and temporalis muscle with intracranial extra-axial extension, pathologically proven as adenoid cystic carcinoma of the lacrimal gland.
