There is a well-defined lesion with septated cystic/necrotic changes seen in the expected location of the right adrenal gland, measuring about 3.2 x 3.2 cm, without detectable internal vascularity, highly suggesting adrenal hemorrhage.

Case Discussion
Waterhouse-Friderichsen syndrome is a cause of non-traumatic adrenal hemorrhage as a consequence of septicemia.
The most common causative organism is Neisseria meningitidis; however, it can occur secondary to Escherichia coli infection.
