Abdominal discomfort.

Gender: Female

Well-circumscribed heterogeneous hyperechoic right adrenal mass.

Well-circumscribed ovoid right adrenal mass measuring 50 x 45 x 30 mm. It elicits an inhomogeneous high signal on T1 GE in phase with areas of signal loss on out of phase, high signal on T2 with no enhancement on postcontrast sequences.

Case Discussion

Adrenal myelolipomas are rare, benign tumors of the adrenal gland, usually asymptomatic and discovered incidentally (as in this case) characterized by the predominance of mature adipocytes. 

The typical adrenal myelolipoma has macroscopic fat-containing components. The amount of fatty component is variable, ranging from only a few small regions in an otherwise mostly soft tissue density mass
