Presentation

Fell from a moving donkey cart.

Patient Data

Age: 35 years
Gender: Male

Anterior dislocation of the bilateral C7-T1 facet joints with narrowing of the spinal canal at the same level consistent with bilateral locked facet joints.

Facture of the superior endplate of T1 with relative preservation of the inferior endplate and no involvement of the posterior wall consistent with compression (wedge) fracture.

Case Discussion

Bilateral locked facet joint is an unstable facet injury and is associated with neurological deficit from spinal canal stenosis as in this case. There are three types of facet joint injuries and they are; disrupted, perched and locked. These types of injuries represent different degrees of dislocation or jumping of the inferior articular process over the superior articular process of the vertebra below. Locked facet is the worst of the three types of injuries.
