Diagnosis probable

Presentation

Known case of acute lymphoblastic leukemia (ALL); completed chemotherapy 1.5 years ago. The patient currently complains of mild lower limb weakness.

Age: 30 years
Gender: Male
mri

Multiple areas of altered signal are noted over the entire vertebral column, involving the posterior elements, with complete suppression on STIR and no post-contrast enhancement, likely suggestive of areas of fatty deposition which could be sequelae to marrow reconversion or post-treatment changes.

The spinal cord appears normal in signal intensity, with T1/T2 hypointense soft tissue seen surrounding the conus medullaris and cauda equina nerve roots within the thecal sac, with no significant post-contrast enhancement, likely suggestive of epidural fibrosis.

Case Discussion

Altered signal intensities getting completely suppressed on STIR without post contrast enhancement likely suggestive of marrow reconversion.
