Diagnosis almost certain

Presentation

Upper abdominal pain.

Patient Data

Age: 20 years
Gender: Male
mri

Multi-focal cystic dilatation of intrahepatic biliary ducts is noted.

Some hepatolithiasis is seen in the dilated biliary ducts.

Prominent upper common bile duct (1.4 cm in diameter). The lower common bile duct measures 0.6 cm in diameter. No obstructive lesion.

No dilatation of the pancreatic duct.

No dilatation of the gallbladder.

Bilateral renal tiny calcifications are seen.

Cystic dilatation of the intrahepatic ducts is noted with echogenic shadowing material inside them.

Bilateral renal tiny calcifications are seen.

Case Discussion

The overall features are suggestive of Caroli disease in association with evidence of medullary sponge kidneys.
