Cerebral dural venous sinus thrombosis

Presentation

Severe sudden headache

Patient Data

Age: 60 years
Gender: Male

​There is an unusual increased density of the left and proximal right transverse dural venous sinuses. Please note the large arachnoid granulations in the distal right transverse sinus.

There are extensive filling defects involving the dural venous sinuses, the vein of Galen and the proximal left IJV. Good opacification of the cortical and deep cerebral veins as well as the right proximal IJV is noted.

Case Discussion

Findings are consistent with an extensive venous sinus thrombosis.

Venous sinus thrombosis is a potentially life-threatening emergency that requires an early diagnosis and treatment to avoid the venous ischemia that it can cause. In our case, there is an unusual increased density of the dural venous sinuses on the non-enhanced CT brain. While it is not a specific sign and can be seen in many other conditions, it is suspicious and warrants further investigation by CT venogram while the patient is still on the table to confirm the diagnosis.
