C5-6 right posterolateral disc herniation with subligamentous extension and caudal migration in keeping with disc extrusion. It is indenting the right anterolateral aspect of spinal cord with mild spinal canal stenosis and encroaching upon right C6 exiting nerve root.

Disc extrusion is diagnosed when there is annular tear and expulsion of disc material with a narrow neck but preserved connection with the parent disc, usually with caudal or cranial migration. It is more common in lumbar disc herniations than in cervical ones.
