A linear hypointense band like lesion is seen within the spinal canal at the posterior aspect of cervicomeddullary junction near occipital bone and posterior aspect of C1 vertebra, significantly displacing and compressing the cervicomeddullary junction anteriorly with changes of myelomalacia.
This most likely represent hypertrophied transverse occipital ligament.
