Presentation

Hearing loss and recurrence of otitis media after surgery.

Patient Data

Age: 30 years
Gender: Male

The large area of signal abnormality in right mastoid-middle ear complex with diffusion restriction on DWI/ADC sequences is in favor of cholesteatoma recurrence.

Partial opacity at left mastoid air cells also is seen.

Case Discussion

This patient had the history of the right ear surgery two years ago and histopathology confirmed right middle ear cholesteatoma. Recently the patient complained of hearing loss and recurrence of right ear otitis media for 2 months.

MRI has an important role in the diagnosis of cholesteatoma and follow-up of postoperative patients for differentiating residual/recurrent cholesteatoma from granulation/inflammatory tissues.
