Dermoid cyst with ovarian torsion

Presentation

Lower abdominal pain.

Patient Data

Age: 35 years
Gender: Female

There is a well-defined right adnexal lesion with macroscopic fat, suggesting ovarian dermoid cyst.

There is a well-defined left adnexal lesion with internal incomplete septation.

Mild pelvic free fluid.

The right ovary appears midline in location, occupied by a heterogenous lesion that appears mainly hyperintense on T1 and T2 with signal suppression on Fat saturated images, associated with whirlpool sign of the right ovarian vascular pedicle, suggesting ovarian dermoid cyst with torsion.

The left ovary shows a well-defined lobulated lesion with septation, that appears hyperintense on T1 with T2 shading, suggesting ovarian endometrioma.

Both ovaries appear abutting each other, giving kissing ovaries.

Mild pelvic free fluid.
