Disseminated peritoneal hydatidosis

Presentation

Patient complains of generalised abdominal discomfort.

Patient Data

Age: 45 years
Gender: Female

Extensive, innumerable, variable-size cysts are seen in peritoneal and pelvic cavities scattered everywhere in the peritoneal cavity, most with multiple internal septations and daughter cysts, some with focal peripheral calcification, the largest of which is up to 10 cm in diameter. There is mass effect on some abdominal structures, including the right kidney, uterus (displaying it superiorly and to the right side), as well as both mid-ureters with secondary mild to moderate bilateral hydroureteronephrosis and no ureteric stones.

Multiple variable-size cysts in the liver are distributed in both hepatic lobes; there is no intrahepatic biliary radicle dilatation; and CBD is normal.

Spleen is seen with well-defined cyst exophyting from its upper pole.

No free fluid collection.

Case Discussion

Findings are consistent with extensive disseminated peritoneal hydatidosis and multiple hepatic hydatid cysts.

Hydatid disease is a worldwide zoonotic disease caused by the larval stage of Echinococcus granulosis (dog tapeworm).

Peritoneal hydatid cysts are seen almost always secondary to hepatic disease. They are usually secondary to traumatic or surgical rupture of hepatic cysts.
