Epiphrenic diverticulum

Case contributed by Mohammad Taghi Niknejad
Diagnosis certain

Presentation

Dysphagia.

Patient Data

Age: 45 years
Gender: Male
Fluoroscopy

Transient marginal irregularity is present in the distal half oesophagus suggestive of motility disorder.

A small epiphrenic diverticulum is also seen before the lower oesophageal sphincter.

There are no typical signs in favour of achalasia. However, contrast media stasis is present even in a delayed 5-minute image.

Case Discussion

Epiphrenic diverticula are pulsion diverticula of the distal oesophagus, less frequent than traction mid-oesophageal diverticula. A single contrast oesophagogram, including prone right anterior oblique views of the distal oesophagus, is the best imaging method for diagnosis.
