External angular dermoid cyst

Presentation

Swelling at the right lateral orbit.

Patient Data

Age: 2 years
Gender: Male

A well-defined cystic lesion is noted in the subcutaneous plane of the lateral margin of the right orbit and measures about 18 x 17 x 10 mm. It has smooth, thin walls with no soft tissue components or calcifications. No fat component seen within. There is no deep intra-orbital extension. The underlying bone and adjacent lacrimal gland are unremarkable. 

Brain parenchyma appears unremarkable.

Case Discussion

Orbital dermoid cysts are the most common orbital masses in children. It can be located either in external or internal angular. It also can be superficial, deep, or a combination of both. The presence of fat components will increase diagnostic confidence.

The less likely differential is an epidermoid cyst.
