Recurrent GI bleeding, neutropenia with a history of rheumatoid arthritis for 15 years

Enlarged spleen with evidence of portal hypertension complicated by gastric and esophageal varices. There are no signs of liver cirrhosis or portal vein thrombosis.

Case Discussion

Felty syndrome is a rare complication of rheumatoid arthritis, which occurs in less than 1%. It is characterized by the presence of three clinical features: rheumatoid arthritis, splenomegaly, and neutropenia. Sometimes Felty syndrome can cause non-cirrhotic portal hypertension.
