Presentation

Bilateral inguinal pain - few days.

Patient Data

Age: 60 years
Gender: Male
ultrasound

Long-axis image of the right groin during the Valsalva manoeuvre shows protrusion of the peritoneal fat inferior to the inguinal canal. It extends to the caudal direction. Short-axis image reveals herniated fat lying medial to the femoral vein. No fat herniation at the rest.            

Similar fat herniation on the left side during the Valsalva manoeuvre.

Bilateral post inguinal hernioplasty status. No inguinal hernia recurrence. No collection/ mass lesion in the inguinal regions.

Case Discussion

The patient with bilateral inguinal hernioplasty about four months prior to the presentation. Complaining of bilateral inguinal mild pain (more on the right than left side) for the last few days.

The ultrasound shows expected findings of post-inguinal hernioplasty status without inguinal hernia recurrence. However, there are bilateral reducible femoral hernias that were clinically suspected.

The risk of femoral hernia is increased after inguinal herniorrhaphy1. However, the present case is with a recent inguinal hernioplasty, not the herniorrhaphy.
