Femoral neck stress fracture

Case contributed by Dr Roberto Schubert

Presentation

Left hip pain in a physically active child.

Patient Data

Age: 12 years
Gender: Male
MRI

The left femoral neck shows marrow edema extending to the subtrochanteric region. There is a medial medullary sclerotic line reaching the medial cortex of the femoral neck consistent with a stress fracture. It shows low signal intensity on T1 and T2 with mild irregularity.

CT

Cortical fissure at the medial aspect of the femoral neck surrounded by a sclerotic band and edema.

Case Discussion

Stress fractures in children are rare. Osteoid osteoma would be the main differential in this region.
