Presentation

Fainting attack.

Patient Data

Age: 60 years
Gender: Female

Widening of brain sulci, Sylvian fissures and basal cisterns reflecting involution brain changes.

Multiple white matter high T2/FLAIR hyperintense foci in periventricular white matter and coronal radiata suggestive of mild chronic white matter ischaemia.

The left posterior communicating artery is enlarged supplying the left posterior cerebral artery and hypoplastic P1 segment of left posterior cerebral artery, known as fetal origin of posterior cerebral artery (Fetal PCA) as a normal variant.

Case Discussion

Fetal origin of the posterior cerebral artery is a common normal variant where the internal carotid artery is the main supplier of the posterior cerebral artery, seen with a hypoplastic or absent P1 segment of posterior cerebral artery.
