Fibrous dysplasia - sphenoid bone

Diagnosis probable

Presentation

Mild right proptosis.

Patient Data

Age: 40 years
Gender: Male

Bone expansion with ground-glass density matrix is seen involving the body of sphenoid bone, right greater wing of sphenoid bone, and right pterygoid process. This exerts mass effect in the form of narrowing of superior and inferior orbital fissures as well as mild right proptosis.

Case Discussion

The ground glass bone matrix is suggestive of d fibrous dysplasia. Involvement of greater wing of the sphenoid bone can lead to narrowing of optic canal, superior and inferior orbital fissures which can give symptoms of orbital apex syndrome. The differential diagnosis is an intraosseous meningioma.
