Presentation

Fall on the ground resulting in right thumb injury one week ago. Thumb base region pain.

Patient Data

Age: 50 years
Gender: Female
ultrasound

Full-thickness defect involving the distal end of the ulnar collateral ligament at the thumb metacarpophalangeal joint. 1st cine-loop shows the defect as an echogenic line due to non-bony avulsion of the ligament. The torn ligament is located deep to the adductor aponeurosis. 2nd cine-loop shows normal gliding of the aponeurosis over the torn ligament during interphalangeal joint flexion-extension. 3rd cine-loop during controlled stress shows joint space widening.

Case Discussion

The patient presented with a history of right thumb trauma. Initial radiographs were negative for a fracture (not uploaded, no copyright). The clinical examination revealed an unstable metacarpophalangeal joint with possible ulnar collateral ligament injury. The ultrasound shows a full-thickness tear of the distal end of the ulnar collateral ligament which was treated conservatively. There was no Stener lesion which requires surgical treatment.
