Presentation

Left lower abdominal pain. History of cesarean section 2 years prior.

Patient Data

Age: 40 years
Gender: Female
mri

Large well-circumscribed ovoid mass in the left upper pelvic region of intermediate to high signal on T1 and high signal on T2, containing serpiginous structures of intermediate to low signal on T2, with peripheral enhancement on postcontrast sequences (granuloma). No pelvic lymphadenopathy or free fluid in the peritoneal cavity.

Normal appearance of the uterus and ovaries.

Case Discussion

The clinical history and MRI features are most consistent with a gossypiboma that was confirmed at surgery (laparoscopic approach) as a surgical gauze.
