Haemangiolipoma - ischioanal fossa

Diagnosis almost certain

Presentation

Painless swelling of the left buttock progressively increasing in size with cutaneous ulcerative lesion.

Patient Data

Age: 35 years
Gender: Male

Relatively well-defined lobulated soft tissue mass centred on the left ischioanal fossa. It shows two components, a fatty component of high signal on T1 and T2, attenuated on fat-suppressed sequences and a soft tissue component isointense to muscles on T1, slightly hyperintense on T2 and STIR, containing foci of calcification (phleboliths) with vivid heterogeneous enhancement on postcontrast sequences. A mass effect is noted on the prostate, rectum and canal anal which are displaced to the right. No extension to the adjacent soft tissue structures.

Case Discussion

MRI features of a soft tissue mass with two components, a fatty component attenuated on fat sat sequences and a hypervascularised soft tissue component.

The patient underwent an ultrasound-guided biopsy with a histopathological exam that confirmed the diagnosis of haemangiolipoma.
