There is a small well-circumscribed lobulated lesion in segment 3 of the liver measuring (18 x 15 x 12 mm) with a homogeneous fat content (density = -76 HU) and no enhancement on postcontrast images. No other hepatic lesion is seen. The rest of the CT exam is unremarkable.

Case Discussion

Small well-circumscribed hepatic lesion of a homogeneous fatty-density with no enhancement suggestive of a hepatic lipoma (incidental finding). It is considered an uncommon benign lesion.

On imaging, the main differential diagnosis is an angiomyolipoma which remains difficult to differentiate from a lipoma due to the lack of pathognomonic features and often the final diagnosis is given by biopsy or resection
