Generalized cerebral volume loss with extensive chronic small vessel ischemic change and ex vacuo dilatation of the ventricles.
Numerous small susceptibility foci scattered through the cerebellar hemispheres, brainstem, basal ganglia, thalami and cerebrum most consistent with chronic hypertensive encephalopathy.
Case Discussion
The key finding of hypertensive microangiopathy is the distribution of microhemorrhages affecting the basal ganglia, pons and cerebellar hemispheres.
