Hypertrophic olivary degeneration

Previous pontine bleed (6 months ago)

Patient Data

Age: 55 years
Gender: Male

A region of hemosiderin staining with the blooming artefact is seen on the right side of the dorsal pons, consistent with a prior hemorrhage, probably from an underlying cavernoma. The ipsilateral inferior olivary nucleus demonstrates striking increase in T2 signal and is swollen. No contrast enhancement. 

Features are consistent with hypertrophic olivary degeneration.

Typical appearances of hypertrophic olivary degeneration. It is important not to mistake this for a more sinister pathology such as a tumor or demyelination.
