Hypopharyngeal squamous cell carcinoma

Presentation

Cough, dysphagia to liquids.

Patient Data

Age: 65 years
Gender: Male

Tumour centred in left pyriform sinus. Cranially begins at lateral glossoepiglottic fold, laterally infiltrated hyoid bone, loss of fat planes with submandibular gland, spreads down the paraglottic fat, involves left false and true cord, extends into anterior and posterior commissure with breach of left cricoid cartilage. No post cricoid /subglottic extension. 

Two left level Ib and one level II malignant lymph nodes largest 9 mm in short axis.

Case Discussion

Histopathology: tissue fragments covered by stratified squamous epithelium with infiltrating neoplasm composed of sheets and nests of cells having scant to moderate cytoplasm and markedly pleomorphic hyperchromatic nuclei. Many mitotic figures. Stroma shows dense chronic inflammatory cell infiltrate.

Features suggestive of infiltrating squamous cell carcinoma - poorly differentiated.
