Presentation

Patient with known systemic sclerosis with an onset of pain and limited range of motion on her right wrist a few months ago.

Patient Data

Age: 45 years
Gender: Female
x-ray

Lunate sclerosis and collapse (flattening), compatible with advanced Kienböck disease.

Neutral ulnar variance.

Case Discussion

Kienböck disease refers to the avascular necrosis of the lunate bone, which is a rare condition with an unclear etiology.

Some conditions have been reported to be related to a greater risk of Kienböck's, such as systemic sclerosis and lupus.

Although negative ulnar variance is a known risk factor for Kienböck's it is not always observed, such as in the present case, which has a neutral ulnar variance.
