Presentation

New onset seizures. Known breast cancer. Previous MRI 2 months ago - no parenchymal disease. On chemotherapy prior to admission. New brain metastases?

Patient Data

Age: 70 years
Gender: Female
mri

Diffuse nodular meningeal enhancement most marked over the superior cerebellum, mesencephalon and Sylvian fissures. 

5mm left frontal and 5mm left medial temporal lobe parenchymal metastases.

Bone metastases.

Case Discussion

Leptomeningeal carcinomatosis with the zuckerguss (sugar coating) best appreciated over the folia of the superior cerebellum.
