Medial plica

Presentation

Anterior and medial knee pain. History of arthroscopy, no trauma.

Patient Data

Age: 30 years
Gender: Male
MRI

Retropatellar cartilage tear covering less than half of cartilage thickness. At this level there is a medial plica extending from the medial side of the joint into the patellofemoral compartment. No cartilage defects elsewhere.

Intact cruciate, collateral and other ligamentous structures. No bone marrow edema. Normal menisci.

Subtle focus of high signal intensity on T2 fat sat images superolaterally in Hoffa's fat pad with no evident patellar subluxation.

Case Discussion

Plica medialis.

Superolateral impingement.
