Presentation

Female with history of pulmonary adenocarcinoma started with two week instability while walking and early morning headache.

Innumerable tiny nodular lesions are seen throughout the supra and infratentorial brain.

Case Discussion

Most of brain metastases are macroscopic masses typically located at the grey-white matter junction. Miliary metastases are less common but possible, most of them are seen on patients with lung carcinoma.

When managing a patient with neoplastic history and neurological symptoms it is important to consider an MRI study looking for miliary metastases even if there are no visible  lesions on the CT.
