A 4cm defect is present in the anteromedial aspect of right hemidiaphragm that some omental fat herniated through it into the right hemithorax causing mild adjacent parenchymal collapse. In addition, the right hemidiaphragm is elevated and eventration is noted. There are also several atelectatic bands scattered bilaterally.
