Mucinous adenocarcinoma in gallbladder with fundic duplication

Presentation

Right upper quadrant pain.

Patient Data

Age: 40 years
Gender: Female

The gallbladder shows duplicated fundus with a thick septum (8 mm). A small vascularised echogenic mass is noted, attached to the septum. 

The MRI sequences confirm the duplicated fundus of the gallbladder with an enhanced soft tissue mass attached to the septum. No extension to the adjacent segments of the liver (IV/V).

Normal appearance of the intra-and extrahepatic biliary ducts as well as the solid intra-abdominal organs. No abdominal lymphadenopathy is seen.

 

Case Discussion

The ultrasound and MRI features are suggestive of a tumoural process in a in a gallbladder with a duplicated fundus type B according to the Boyden classification of congenital duplication of gallbladder 1.

The patient went on to have a cholecystectomy with a histopathological exam which confirms the diagnosis of mucinous adenocarcinoma of the gallbladder with duplicated fundus.

Mucinous adenocarcinoma of the gallbladder is considered an uncommon variant and account for 2.5-5.5% of GB carcinomas 2,3.
