Multifocal pulmonary stenoses and webs due to chronic PE

Presentation

Multiple previous PEs overseas. Worsening SOB and exercise tolerance. High right heart pressures. Work up for CTEPH.

Patient Data

Age: 75 years
Gender: Female

VQ scan

Nuclear medicine

SPECT of lungs following inhaled technegas and intravenous Tc99m-labeled MAA.

Extensive bilateral ventilation and perfusion mismatch defects in all pulmonary lobes, which has worsened since the previous VQ scan 2 years earlier.

Multifocal webs and strictures within the segmental and subsegmental pulmonary arteries bilaterally. The strictures are more numerous in the right lung and maximal in the right lower lobe, right upper lobe and the left lower lobe.

Case Discussion

The patient was referred to another hospital for balloon pulmonary angioplasty.
