Olecranon fracture - occult

Presentation

Fall. Elbow pain and reduced range of motion.

Patient Data

Age: 6 years
Gender: Female
x-ray

Joint effusion with anterior fat pad sign present. No fracture is identified. Skeletal alignment is within normal limits.

Moderate joint effusion. Soft tissue hyperintensity overlying the olecranon. Marrow edema within the olecranon with a low signal fracture line extends to the articular surface, best seen on the sagittal PD sequence. 

Case Discussion

In the setting of trauma, an elbow joint effusion is highly suspicious for a fracture. In children, the most common site of occult fracture is the supracondylar region. This patient received an MRI because the supracondylar region was "minimally tender" and there was concern for fracture at another site.
