There is a well-circumscribed ovoid intra-orbital soft tissue mass of intraconal location in the left orbit. This lesion is in contact with the posterior border of the globe and inferior rectus muscle with mass effect on the optic nerve, which is displaced superiorly.

This mass shows intermediate-high signal on T1 and inhomogeneous high signal on T2/FLAIR sequences, with a pseudocapsule. Grade I proptosis is also noted.

Case Discussion

MRI features are most consistent with an orbital cavernous hemangioma, also known as cavernous venous malformations of the orbit, which is considered as the most common vascular lesion of the orbit in adults.
