Osteochondritis dissecans - lateral femoral trochlea

Presentation

Left knee injury a few weeks ago.

Patient Data

Age: 20 years
Gender: Male

Intra-articular body within the suprapatellar recess with cortical irregularity of the femoral trochlea. Small joint effusion. 

Moderate joint effusion. 

Chronic osteochondral defect of the lateral femoral trochlea with the fragment displaced into the medial suprapatellar recess. The osteochondral defect has a sclerotic rim and underlying high signal. Chondrosis of the adjacent cartilage. 

No acute meniscoligamentous injury. 

Case Discussion

This is an example of stage IV osteochondritis dissecans with a displaced intra-articular fragment (stage IV). The femoral trochlea is a less common site in the knee.
