Osteochondroma

Presentation

Right lower thigh swelling and pain.

Patient Data

Age: 20 years
Gender: Male

A bony lesion is seen protruding from the right distal femoral metaphysis, being continuous with the bone, it shows the same signal intensity as the femoral bone. It also shows an irregular overlying margin and thick cartilaginous cap measuring about 1.7 cm and is seen indenting the posterolateral muscles of the right thigh.

Case Discussion

Osteochondromas most commonly arise from the appendicular skeleton especially around the knee. MRI is the best modality to assess the thickness of the overlying cartilaginous cap which usually elicits bright T2 and low T1 signal intensity.

An irregular, thickened cartilaginous cap may denote development of chondrosarcoma on top of osteochondroma which is usually of low grade.

Osteochondromas may be single or multiple (hereditary multiple exostoses) and appear either sessile or pedunculated.
