Osteoid osteoma - spine

Presentation

Persistent mid-upper dorsal pain

Patient Data

Age: 20 years
Gender: Female

The MRI sequences demonstrate:

  • Mild dorsal scoliosis of left sided convexity
  • a small relatively well-defined ovoid lesion of the right lamina of T3 of low signal on T1WI/T2WI, representing most likely the surrounding sclerotic reactive bone. The central area (3 mm) of high signal on T2WI with mild enhancement represents most likely the nidus.
  • edematous infiltration with enhancement of the surrounding bone marrow and right paravertebral muscles. 
  • note that the lesion is located on the concavity at the apex of the curve.

The annotated images (axial T2WI and postcontrast T1 fat saturation) indicate the location of the lesion within the right lamina of T3.

Case Discussion

MRI features of a lesion of the right lamina of T3 representing probably an osteoid osteoma.

The nidus, as well as the central mineralization, are better demonstrated on CT than on MRI, however, the bone marrow /soft tissue edema is well-visualized on MRI than CT.
