Ovarian mature cystic teratoma with floating balls sign

A large cystic lesion occupies the pelvis with multiple globules inside, with a signal higher than fluid at T1W and lower than fluid at T2W, restricted diffusion, and a drop in signal at the out-of-phase sequence, indicating a fatty component. There was no significant enhancement of the globules.
Case Discussion
The floating balls sign is an uncommon but pathognomonic sign of a mature cystic teratoma of the ovaries and refers to the appearance of multiple spheres of solid, usually fatty tissue within a cystic adnexal lesion.
