Presentation

Lower abdominal pain.

Patient Data

Age: 35 years
Gender: Female

Transvaginal

ultrasound

The right ovary appears enlarged, avascular, midline in location with central echogenic stroma and cystic changes suggestive of ovarian torsion and central necrosis.

The left ovary shows a cyst and otherwise appears unremarkable.

The right ovary shows highly attenuating stroma and appears midline in position with surrounding mild amount of abdominopelvic free fluid indicating hemorrhagic infarction of the right ovary.

Intraoperatively, the right ovary appears black in color, indicating infarction.

Case Discussion

This case was referred from another hospital as a case of nonspecific severe lower abdominal pain of 1-day duration. The patient stated that there is a gradual decrease in the abdominal pain on presentation.

Transabdominal ultrasound shows an enlarged midline right ovary with minimal arterial pulse wave activity. Transvaginal ultrasound shows completely avascular right ovary with central echogenic stroma and cystic changes, and the diagnosis of right-sided ovarian torsion was established.

Unfortunately, during the operation, the ovary appeared infarcted and was resected.
