Presentation
Farming accident - tree vs tractor.
Patient Data
>75% fibre disruption (defect measuring up to 2 cm transverse and 4-5 cm craniocaudal) of the distal muscle belly extending to the distal myotendinous junction with torn fibres folded distally; the distal free tendon is intact and attached to the proximal humerus but is mildly high signal and oedematous. Adjacent fluid and haematoma.
Subcutaneous oedema in the anterior chest wall subcutaneous tissues. Myofascia oedema along the anterior surface of both the short head of the biceps and anterior deltoid muscle bellies.
Case Discussion
This is an example of a non-insertional pectoralis major muscle injury with the torn fibres primarily in the muscle belly but extending into the myotendinous junction.
