Multiple venous varicosities are seen in the left adnexa extending into the uterus and vagina feedfed by a dilated left ovarian vein arising from the left renal vein.

A 46×36 mm fat containing-containing mass is noted aton the right ovary most likely consistent with a dermoid cyst.
