Periventricular leukomalacia - end-stage

Diagnosis almost certain

Presentation

Developmental delay. History of a perinatal insult. Had VP shunt for hydrocephalus (surgical report not available).

Patient Data

Age: 5 years
Gender: Male

The MRI sequences demonstrate:

  • ventriculomegaly with irregular margins of the bodies and trigones of the lateral ventricles

  • loss of periventricular white matter with increased FLAIR and T2 signal

  • thinning of the corpus callosum

Case Discussion

The clinical presentation and the MR findings are most consistent with end-stage periventricular leukomalacia (PVL).
