Petroclival meningioma

Presentation

Right-sided trigeminal neuralgia, chronic headache, and vision problems.

Patient Data

Age: 55 years
Gender: Female
mri

There is a right-sided petroclival meningioma with long enhancing dural tail, adjacent vasogenic oedematous changes, and midline cross associated with extension and involvement of adjacent structures as followed:

Invasion with effacement of right Meckel cave and cavernous sinus with extension to the contralateral cavernous sinus

Encasement of right ICA lacerum and cavernosal segments with marked narrowing and occlusion

Invasion of the mass to the right-sided optic canal and superior orbital fissure with optic nerve encasement associated with mass effect causing proptosis

Superior extension of the mass into the middle intracranial fossa causing pressure effect on the inferior aspect of the right temporal lobe and diencephalon structures 

High signal foci in T2 and flair sequences at subcortical and periventricular white matter of both cerebral hemispheres depict microvascular ischaemic events

Case Discussion

Petroclival meningiomas had a considerable high postoperative morbidity and mortality because of tumour proximity to cranial nerves, major blood vessels, and the brainstem.
